You are too early for a facelift at this time. Your perioral anatomy is in very good shape. At this point in your subtle aging process, I would recommend daily SPF and skin resurfacing technologies to boost natural collagen production (ie: RF microneedling devices, CO2 laser, etc.).

I am taking out all textured implants in patients who consult with me who have had them previously placed from other surgeons. I do not place textured implants, nor do I recommend them because of their association with ALCL. I would recommend smooth round silicone implants placed sub muscularly.

From your photos, it does not looks like a facelift at this time would benefit you. An in-person consultation with assessment of your skin laxity would be necessary to know for sure.
Consider doing skin resurfacing and collagen boosting technologies to keep skin thicker and more youthful. CO2 laser, RF microneedling, etc.

You will need to choose a moderate to larger sized implant to fill out breast envelope and raise NAC's. A high profile implant will aid in doing that and also provide more upper pole fullness. It is important that you try on the 500cc range implant sizers to make sure that you are comfortable with your size choice. If you drop down to Mod Plus, you will lose some upper pole fullness and would likely need to drop down to 425-450cc volume range. The choice is ultimately yours to make.

The best way to feel comfortable about your implant size choice is to be a very active participant in your size choice. In our clinic, we have all patients try on sizers to find the volume that they are liking. We are a team in making sure that the measurements and profiles are correct and safe choices, but you as the patient have the highest success rate if you play an active role in the decision making.

Thank you for providing photos. It is, however, difficult to assess how much skin laxity (excess skin) that you have. An in-person consultation will be best to determine if you have enough laxity for an abdominoplasty.

In my clinic, I do not offer breast augmentation and lifts at the same time. There is a high associated complication rate by doing these two procedures together. I would advise that you get a mastopexy, or breast lift first. This alone will greatly improve your breast aesthetics. After you heal from a lift, you can decide if you still desire more breast volume and place an implant.

It is always advisable to do skin removal surgeries when you are at your ideal height and weight with no planned weight gains/losses. You are at a higher risk of developing more skin laxity after your tummy tuck (throughout your future) simply because you have undergone a significant weight loss already (your skin has lost some natural elasticity). Thank you for the question and photos provided. It does appear that you have some tissue falling below the inframammary fold but your anatomy is still in good shape with nipple areolar complexes at nearly the maximally projecting point on the breast. If you are wanting more breast volume and added upper pole fullness, you could consider an implant alone to fill out your breast envelope and give you an appearance of a slightly higher, fuller breast.

It is common to be conservative with correcting asymmetries considering the silicone implants are made in only 25cc (50cc in larger implants) increments. It is extremely important that you are apart of the decision-making process with placing same or different sized implants when you do have a size discrepancy. We have all of our patients try on sizers, same sized and then adding the 50cc sizer to smaller side. This can quickly let you know if this corrects the asymmetry or if it is too much...
