Kybella is great for getting rid small unwanted pockets of fat. It has FDA approval for the submental (chin). People are using it in other areas off label as well, so make sure you have a provider who is skilled and comfortable treating off label areas.

Kybella is great for getting rid of unwanted fat. Your submental area looks great, there is not a distinguishable bulge or pocket. We would discourage you from doing Kybella and gear you toward a radio frequency skin tightening procedure, perhaps Endymed 3Deep or Thermage. We like to see an easy to distinguish pocket or bulge of tissue (fat) under the chin. Kybella is a series a 4 treatments done every 6 weeks, with some downtime. This is a dynamic laugh line, very hard if not impossible to completely get rid of. Everytime you smile, it is there. Your best treatments would be a combination of resurfacing, with either the 1540 or RF Microneedling Intensif to resurface the texture of the skin. You could put a little filler into the cheek wrinkle but be careful and use Belotero or Volbella but it’s less the product and more important to choose a provider who uses many different fillers. A bit fillers in the mid face (malar fat pad) will help the lower face by gently lifting the lower face. Yes you could do some fillers in the corner of the mouth to add some structure and to hold up the corners of the mouth and help stop the downward tug. A drop of botox in the DAO (depressor anguli oris) will help neutralize the corners of the mouth and give a slight upward tilt.

You have a lovely cupids bow! Projection problems will not be fixed by adding more filler. You might have had a bit of filler migrate or they might have done the vermillion border. To remedy over projection try the threading or tenting technique which deposits the filler in a way that will help draw the lip upward, not out. It’s usually not the product that causes problems, rather technique.

You might want to try a little botox on the upper lip. There are 2 things to think about. You could try the treatment for Gummy Smile as it will help relax the upward tug and drop the inadvertent elevation. If that doesn’t offer relief, consider the Lip Flip, a subtle relaxation in the upper lip that can roll up the vermillion border and soften the muscles of the upper lip.
