This is a fun area to treat as it attempts to restore a more youthful appearance of the jawline. As we age, the jawbone thins out and elongates. Filler can restore the appearance of the thickness of the jawbone. It is considered a more advanced technique and definitely better with a cannula. I have seen this before. It is most likely from BBL. The area directly over the breastbone is at increased risk for thermal injury (just like the forehead). I am very hopeful it will heal…but may take about six months. If there is any residual discoloration after that time, then one can use some other treatments to reduce the discoloration. A very good chance it will go away entirely.

Coolsculpting involves permanent loss of fat cells, while Emsculpt builds muscle and reportedly causes fat loss. Emsculpt requires regular follow up treatments…it is not a permanent solution. There is alot more published research on Coolsculpting and it has been around alot longer. Also, realize that several competitors to Emsculpt will be coming out in the next year or so. The price is likely to drop when there is more competition.

Eyelids are a very sensitive area with limited blood supply. It is definitely not an area for fillers. The nose is an extremely risky area. I would give it a total of 14 days… it should be mostly gone by day 7. Since it is a firmer filler and is placed deep, it can be tender and firm. If the area is blanched (white) or purple, it needs more immediate evaluation. Oral arnica may help. Topical arnica is not as effective.

Thank goodness for lidocaine! As a physician, I can’t imagine a world without it.
For the most part, the vast majority is out of your system (via your kidney) in a few hours. If you want to be official, the half life of lidocaine is 1.5 hours. If you multiple this by 7, it takes about 10.5 hours for it to be nearly entirely excreted. The effects are long gone by then. Other things can delay lidocaine removal like liver disease.

You have a variety of scars–some more icepick and others deeper. There is probably no single treatment that will take care of everything. The good news is you have a variety of options:
1. Bellafil with subcision–this might be the best single treatment based on the photos
2. Microneedling combined with PRP. We have had fabulous results with this.
3. Resurfacing–can be fractional resurfacing with less downtime. It does not necessarily need to be a full face…

I would not recommend using a wet or dry Sauna so close to an IPL. Your skin remains in a healing phase and is vulnerable to thermal injury. Think of IPL as a “controlled burn”–you wouldn’t want to add intense heat after a burn. Also, you want to avoid/minimize flushing if the IPL treatment was used to treat red vessels. Let everything settle down….I would recommend one month.

The vast majority of brown spots are benign, but I would agree seeing a provider to make sure the spots are nothing more. However, it does not require a dermatologist to do this. These spots are easily treated with phototherapy. We use Sciton BBL and it works beautifully for this (all mine are gone!). There is no down time and it is inexpensive and well-tolerated.

I would agree it does not look like MENDS. It is difficult to tell from the photos that you provided, but it could be some post treatment blisters as your doctor says. Or, it could be some oozing from the locations if you had alot of pinpoint bleeding.

Should be no problem and may provide some relief from the itching and swelling some experience. Also, I am not aware of any firm evidence that anti-inflammatory medications like motrin well lessen the results. The freezing programs the cell to die over the next few months. While we prefer Tylenol, some people really hurt after Coolsculpting and a small amount of motrin can help. Most aesthetic physicians do not recommend prescribing narcotics.

I would say about 50% of our patients notice no or little change after their first round of Coolsculpting. However, when we show them the before and after photos, they can see the changes and are happier. When you look at the treated area everyday, you will not notice the change.
