After reviewing the photos, it looks like liposuction woudl improve this area very well. I frequently will liposuction this area in patients after breast augmentation or at the same time to improve this. Having liposuction in the anterior axilla and chest will not expose your implants more, it will however improve your contour. Sometimes it is necessary to add a skin tightening procedure to this area to improve the skin but this would be determined at an in person consultation.

It looks like you will benefit from a lift at the time of breast augmentation. I do not think you will be happy with just a breast augmentation alone as your nipple position is below the inframammary fold. If an implant is placed, the breast volume will be better but the nipple will still be too low and most patients are not happy with the results if they need a lift and only do the augmentation. It does appear that a labiaplasty woudl improve the appearance of your labia minora and a clitoral hood reduction could be done to reduce the excess tissue over the clitoris as well. As far as painful intercourse, this is sometimes the reason, but there are many other causes that need further evaluation by your gynecologist. These options need to be reviewed as they will have different outcomes but a third option would be to do the lift first and if you decide you want more volume or more upper breast fullness then an implant can be added. If a mini tuck is done even with liposuction, I believe the upper abdomen skin and muscle separation woudl bother you after the procedure. I usually suggest wearing a compression garment that does not compress any area that has had fat grafting for at least 6 weeks. After this there may not be a need to compress the area but this is on a case by case basis. I do suggest wearing a compression garment for the areas where the fat is taken from however as this will help with swelling and final shape.

Congratulations on losing the weight. That is amazing. After looking at your photos, I don’t think it is an absolute that you get a fleur de lis tummy tuck. Most of the time to avoid the scaring centrally and extended tummy tuck can be done to remove the excess skin you have along with tightening the abdominal muscles. You have some skin along the waistline and extending the scar in the bikini line should remove most if not all of that as well.
