Research your doctor and make sure he is a board certified Plastic Surgeon. You can go to plastic surgery.org or the American Board of Plastic Surgery to check any legitimately trained Plastic Surgeon.

In my opinion you would get the best result with a mastopexy or breast lift, as a significant amount of your breast tissue is below the breast crease. Your doctor should review the different types of breast lift procedures with you.

A areola lift would not be adequate in my opinion to give the best result.

Your body art will also need to be considered as it will likely be affected by these decisions.

It is normal to have some selling that persists and worsens throughout the day for the first few months after an abdominoplasty procedure. If you feel that there is fluid moving around , you could have a serous fluid collection that could be aspirated. It would be best to follow up with your surgeon for evaluation.Good luck!

I don't think a reverse TT would be appropriate in your case. This is a surgery reserved for very unique circumstances and most often requires a scar across the midline in the cleavage area which can be objectionable to most patients.A full abdominoplasty would be the best choice based on your photos in my opinion.Good luck!

I think you need to strongly consider placement of acellular dermal matrix such as Strattice or its equivalent to help protect you from a recurrent contracture.Implant choice is less of a factor vs sterile technique, pocket placement, and meticulous hemostasis.Your autoimmune condition may play a role if you are on immunosuppressant medication.Good luck!

If you are not having issues with the sub glandular pocket, you do not absolutely have to undergo a pocket change.

The main indicators for a pocket change include, capsular contracture, and visibility of the implants in the SG space. Implants in the SG vs the SM space do have a higher contracture rate in my experience. Placing a textured device may give some protection in the SG space.

Last option is to leave the saline implants alone- but it sounds like you are being proactive.
