I assume that you are talking about the nasolabial folds for smiles lines. Fillers will help to reduce the lines ont eh face. Juvederm Vollure is a good filler that lasts 1;5 years and would work well in this area.
Another filler that I like for these smiles lines is Radiesse as it very natural in that it contains Calcium hydroxyapetite which helps to not only strengthen bones but helps to stimulated Radiesse production.

Contrast given during an MRI is called gadolinium and is less toxic than the iodinated contrast that is given with a CT scan. Gadolinium is not an issue with fillers and vice versa. Not sure which part of the body you are doing fillers but if you are doing the temples and you go for brain MRI, the radiologist might see the newly inserted filler and might wonder what it is. If this is the case, you would want to do MRI first and then have the fillers inserted.

This type of hollow can easily be filled with filler. I like Juvederm Voluma for this type of volume loss although Radiesse is another option as Radiesse also helps to stimulate collaged development.

Volumizing the mid face (cheekbones) would reduce the gauntness you talk of. Juvederm Voluma is a great product for volumizing the mid face as it lasts two years and is smooth yet firm. If cost is an issue Radiesse can also be used for cheekbones but lasts only a year.
Getting a filler in the smile lines also known as Nasolabial folds will help the gaunt/hollow look you speak of. Juvederm Vollure is good in this area although sometimes more than one syringe is needed to get the job done….

I do not see why the provider did not want to do more filler in the cheekbones. Augmenting cheekbones helps to pull up on smile lines and gives a more V shaped face. If you feel you need more volumizing in the mid face, feel free to seek out another provider.

It is surprising that the Restylane did not dissolve with the hyaluronidase. Ask the provider who did hyaluronidase treatment to dissolve the Hyaluronic acid filler if they used branded hylenex (hyaluronidase) or if they used a compounded version. The branded Hylenex is more expensive but is better than the cheaper compounded version. Ask someone who is experienced in using Hylenex and will use the branded product to dissolve the filler. It should dissolve.

It does not appear that your fillers have migrated. The fillers are within the lips, have not migrated beyond the lips. Your lips appear well filled, are shaped symmetrically and the upper lip is a little bigger than the lower lip like it should be. Enjoy your lips!
