A difference of 20cc in implant size will have minimal affect on your result. The difference is .25cm in the diameter. If you are more concerned about being too small, pick 275cc if your concern is being too big, pick 255. I don’t find bra sizers particularly helpful, what an implant looks like in a bra does not always translate into what it looks like in the body. If you trust your surgeon’s judgement, allow her/hIm to pick the size that most appropriately fills the space.

The procedure for implant removal depends on the implant location, type, tissues, and your goals. This is a very good question. If a patient expresses concern about the capsule and I can remove it safely, I will remove it using a precise dissection. There are potential complications to capsulectomy which you should discuss with your surgeon. Larger implants should give your desired look, but be sure that you understand the trade offs with larger implants. The more weight in the breast the higher the possibility of thinning of the overlying tissues with lateral and inferior malposition. You have a good result now and 500cc implants would probably not cause significant issues, but you want to take into account all possibilities when choosing an implant and I like to do what I can for my patients to have results they will be happy...

You have a nice result from your surgery. Everyone has some degree of asymmetry and a using slight difference in the volume in your implants would not make a big difference and certainly would not be worth the additional risk and expense of a revision. 
I’m schedule for explant and my doctor is not going to be using drains. Is that okay?

I routinely use drains with explant, I think it’s especially important if the capsule is also removed to remove fluid that may collect in the space left from removal and capsulectomy.  I would discuss with your surgeon the reason she/he doesn’t use a drain and how they manage seromas that may occur without the use of a drain.

The only way to treat established capsular contracture is by removing the capsule, especially if it is very thick and calcified. Be sure to find a board certified plastic surgeon who is experienced with implant removal.

When removing a textured surface implant, I try to remove the entire capsule using a precise capsulectomy, where the capsule is removed under direct visualization, so as not to damage surrounding, normal tissue. Sometimes that can be done en bloc, sometimes not. If the entire capsule cannot be removed, a thin capsule should not interfere with mammograms, a thicker, calcified capsule could. Both are reasonable options, although with your very loose tissue, I would side with the surgeon who recommends 2 stages. I would lift and reshape the breast removing some tissue. Your breasts will be smaller but in better position and with a better shape. Often patients find they are happy with just the lift and can avoid having implants with the potential issues of having the weight of the implant in a breast with poor quality tissues with the possibility of stretch with time. 
Hard lumps under skin after liposuction and tummy tuck. What can I do for these?

I personally had liposuction and had firm areas for a few months. When we do liposuction, we can’t completely suction out every since damaged fat cell, so cells in our body “eat” those cells and dispose of them, just like it does with a bruise. It can take 6 months for those areas to soften and eventually disappear. Have your surgeon about your concerns on your next visit.

Everything looks fine for one month post op from implant removal. Nerve pain is common after any surgery and can last for weeks or even a few months. Keep your regular follow up visits with your surgeon and don’t hesitate to call him or follow up sooner if you have concerns.

I would recommend checking with your surgeon, as you have significant asymmetry in your breasts. Unless you had significant asymmetry before surgery, I would be concerned that your saline implant may be leaking. This is not an emergency and does not cause any harm, but should be evaluated by your surgeon.

Wound healing issues are not unusual after a breast lift and what you are experiencing may just be a reaction to the sutures that are in the process of dissolving. However, since you have an implant, it’s better to be safe and infection could cause a significant problem, I would suggest you see your surgeon as soon as possible.
