Hello, if remove implants and don't intend to replace them then the capsule will likely seal down on its own. It would be important to have drains afterward to prevent fluid build up (seroma) that could keep the pocket open. Removing the capsule isn't always necessary but can be beneficial, even partially, in cases like yours. Removing the lateral capsule will help that tissue heal quicker. If there is fat in this banana roll, liposuction is the gold standard for fat removal. My office does a lot of coolsculpting also, and it can be effective in treating the skin. I do not have a high opinion of sculptsure. See a board certified plastic surgeon who does both liposuction and coolsculpting and get their expert opinion. Liposuction will remove excess fat. While different methods can help skin retract, if excess skin is the main problem perhaps skin removal would be more effective for you. Surgical options exist for all of these areas. Non invasive methods include skin tyte (infrared light) which will give moderate tightening of skin, but if you have significant excess skin surgical removal of the skin may be best. Capsulectomy is not necessary if the capsule is very thin and soft, which is likely with saline implants. If the capsule is thickened or calcified (or if you had silicone implants previously( then it makes sense to remove the capsule. The fotos show very ptotic breasts and a breast lift (major mastopexy) would be very helpful. I think you would get excellent results doing a full lift first, letting it heal and then adding the augmentation at a later date once the breast lift has healed.
