I would say that this bump could be handled effectively by either a board certified dermatologist or plastic surgeon.  It may be a candidate for a punch excision, with some stitches, and then send the lesion to pathology.

You may want to consider fractional CO2 laser resurfacing, TCA cross, and/or saline subscission.  What kind of doctor are you seeing.  Make sure you consult with a board-certified dermatologist or plastic surgeon who specializes in treating acne scars.  Good luck!

Certainly, I would say that a fractional CO2 resurfacing would likely help your skin at 70 years of age, but you would need a full skin and facial evaluation to see what other treatments would be helpful for you as well.  Best way to proceed would be to seek an evaluation with a board-certfied dermatologist or plastic surgeon.

Swelling that lasts three months after Botox injection is certainly very unusual.  It is hard to say with certainty that this is even related to you Botox injection.  The best course forward would be to seek a consultation with a board-certified dermatologist or plastic surgeon and see if anything can be done about the swelling.  Also you may want to have future injections performed by one of these types of practioners rather than a PA for optimal results.

Who performed this treatment for you, or did you perform it yourself?   TCA when used for acne scars (such as the TCA cross technique) should be used very focally just inside the scar itself.  It appears you have gone beyond the scar boundaries.  At this point, time, sunprotection, and perhaps hydroquinone will be the keys to recovery.  Best to always seek a board-certified dermatologist when considering any time of skin procedure.

I do not think you will have any problems with sclerotherapy related to underlying hyperthyroidism.  Just make sure you seek treatment with a qualified board-certified physician for best results.
