A breast reduction in your case, if you are trying to go "as small as possible" would likely require an anchor scar because as more breast tissue is removed, there is more extra skin that needs to be removed as well. Personally, I start all my breast reduction with a vertical incision. But the good news is that I have seen collapse like this happen in patients with no obvious pathology or serious condition.
