Coolsculpting and Vanquish are two of the noninvasive procedures available for fat reduction. Each patient needs an individual evaluation to determine which is best for them and some patients are candidates for both. In general, vanquish is better for treating a larger amount of the core at one time, while coolsculpting is better for small discrete pockets of fat.

Coolsculpting for submental fat (double chin, fat under the chin) can reduce the fat pad but is not design to tighten skin. However, given the gradual nature of the fat removal over the 3 months following cool sculpting, the skin does typically tighten. If there is still some laxity, a tightening procedure with radio frequency can be used.

The fine lines oor crow’s feet in your picture typically appear in the late 20s or early 30s. From your photo, it does not appear you are prematurely aging. Treatmetn with Botox is the best way to slow down the development of fine lines and is preventative if started at the first signs of fine lines. Your lines are most likely related to sun exposure. I recommend using daily sunscreen and sun proctive measures whenever possible.

Fine lines on the forehead are best treated with a neuromodulator such as Botox or Dysport. If the upper eyelids are heavy, the treatment needs to be lighter to avoid a “heavy brow.” The remaining fine lines can be treated with superficial fillers, such as belotero or juvederm ultra mixed with lidocaine. A combination of fillers and neuromodulators can be the best approach for many areas of the face.

Total FX and Sculptra are both procedures which cause collagen regeneration. Sculptra will work to replace volume and diminish the lines around the eyes caused by loss of tissue in the cheekbones and temples. Total FX will also help diminish these lines by smoothing the surface. Regardless of which procedure you begin with, I would wait at least 2 weeks between the treatments. I prefer to begin with the laser andthen add the filler. Good luck!

If you are planning to have a fractionated CO2 laser such as ultrapulse ( Active FX and Deep FX), you do not also need to have a non-ablative Fraxel. CO2 lasers have more down time but provide greater improvement per treatment. This laser works well for fine lines around the eyes and deeper lines around the mouth. Fraxel would also work well for these areas, but if using the non-ablative wavelengths, it will require multiple treatments to achieve what one ultrapulse can.
I have chicken pox scars on my face. Is it possible to remove them completely?

CO2 laser can be very effective on chicken pox scars. These are best treated with combination therapy, starting with the Cross method, which is a procedure using TCA to regenerate the collagen in the scar, and then to perform resurfacing laser to smooth the surface. Fractionated CO2 is the safest, most effective treatment for scars. However, it is best used on lighter skin tones because darker skin may loose or gain pigment following the procedure. Taking low doses of prednisone after fraxel laser treatments short term can be safe, and may even be prescribed to reduce swelling immediately after the procedure. The under eye area or tear trough can be filled with Restylane. It is important to be selective with candidates for this filler, because while the filler can cause reflection around the eye to improve and make the eyes appear younger, it can also bring in unwanted water causing swelling. I discourage my patients with bags from proceding with tear trough injections for this reason, and if you experience bags with your allergies, I would suggest you avoid additional volume in this area.

Restylane is a hyaluronic acid filler which can be dissolved with hyaluronidase. Anytime there is an undesired result with an HA filler, it can be reversed within about 24 hours and then touched up one week later.

Sculptra is injected in the form of a suspension. If it does not hydrate properly, it is more likely to cause nodules in the skin. I recommend it be reconstituted at least 24 hours prior to its use, and to be warmed prior to injection. In my practice, we typically have sculpa reconstituted for 2-7 days prior to injection.
