Deoxycholine is undergoing clinical trials, and when FDA-approved, will be available to treat small areas such as under the chin, but most would agree that removing fat from the face may not result in a natural-looking appearance. Studies have shown that fat in the face gives a more youthful appearance. Removing fat is generally not indicated in the face. It is often the repositioning of the fat that is more important. The applicators available currently for CoolSculpting are not suitable to be placed on the cheek to reduce fat in the cheek. Some people think they have fat cheeks but they have an enlarged chewing muscle, the masseter.

CoolSculpting Does Not Work for Cheek Fat
Unfortunately, coolsculpting can’t be used in the management of cheek fat for several reasons. The primary reason is the lack of an available hand piece for this type of procedure. In addition, removal of the cheek fat can result in hollowing of the cheeks, which can be very unattractive.
CoolSculpting Not For The Face

Cheek Fat Keeps You Looking Youthful
While the CoolMini applicator from Zeltiq can be used to freeze and eliminate fat from smaller areas, such as double chins and bra bulges, it is not recommended to remove fat from the face. Generally, facial fat actually keeps you looking young, as full cheeks are associated with youth.
Coolsculpting cannot be used on cheek fat but will soon be used under the chin

Currently, the Coolsculpting procedure is designed only for large areas such as the abdomen, flanks, and back. The applicators or the handpieces will only fit these areas.
In any case, if you are concerned about fat at around the lower cheek region, a relatively quick procedure is “buccal fat removal.” Buccal fat removal is a surgical procedure that excise the fat from the lower cheek region.
The incision will be placed inside the mouth. The benefit is that the result is quick as oppose to Coolsculpting which theoretically takes 2-3 months to see a 25% reduction in fat. (Steve Lee, MD, New York Plastic Surgeon)

Coolsculpting currently is only used for body fat reduction. The current handpieces are too large to work on facial fat including cheeks and chins. CoolSculpting for fat removal on the face is not available at this time. The company is supposedly working on new smaller applicators that can remove fat non invasively in other parts if the body like the neck.
