When people meet and interact with others they are usually upright-so the markings are sometimes done upright. Surgery cannot easily be done in the upright position especially with general anesthesia. The experienced surgeon knows how tightly to pull the tissues to get a natural appearance after the swelling subsides.

An exam will be necessary to determine how much of the shape and size is determined by the skin/ fat and how much of the cartilage and bone. Bone, cartilage and fat can be reduced but skin cannot without producing a scar. Sometimes augmenting the bone or cartilage with cartilage grafting can help give the nose a more defined shape. The dropping eyelid can be repaired. This is called Blepharoptosis. The position of the bony orbit is more difficult to correct but once the eyelid position is corrected the entire eye will look better and move symmetric. I hope this helps you.

It is a smaller picture and blacking out the eyes and face makes it difficult to interpret. How much can be done depends on the thickness of your skin and the position of the cartilages. Noses can be improved by reducing or altering the shape of the bones and cartilages and sometimes adding cartilage for the septum, ears or ribs as necessary. I hope this helps.

not evacuated small hematomas last about three months. Some can become chronic and need surgical
removal. If it was evacuated surgically
and the bleeding controlled, it would rarely come back later. Some fluid collections could be lymph rather
than blood and could be more complicated, again, it depends on the location to

It depends on the style of the implant. If it is a Saline implant-there could be a possible leak which would explain this. Also, did you have any weight gain or possible pregnancy? These could all be possible factors to attribute the change. Once the pocket is formed, it really shouldn’t move or shift. I would recommend imaging and full assessment from your original surgeon.

1 in 10 Rhinoplasty cases require revisions. I would recommend going back to the original plastic surgeon to determine whether the septum and turbinates were addressed during original Rhinoplasty procedure. If not, that doctor should be the one who performs the additional work.

I would really need to know what the issues are and what your nose looks like. Maybe the surgeon feels the expectations are unrealistic and he/she would not be able to accomplish that expectation. In any case you must feel comfortable with any steps you are taking.
