That is concerning to have something pop up like that after 3 months. However, I think you have heard the same thing from just about everyone. Your nipple necrosis fears can be put behind you. This appears to be an irritation under the skin, likely due to an absorbable suture that is working its way to the surface before it is actually absorbed. That is a good question and I would be concerned as well. I would definitely return to see your plastic surgeon to have that area evaluated. IMO, this is likely just a small pocket of fluid that has worked its way to the surface and is likely not a big concern. However, without better photos and a real exam nobody can tell you for sure if this is a legitimate issue or not. I am definitely more concerned by your symptoms than I am about the bruise itself. Based on your photos (which don't always tell the whole story), the nipple and areola seem well below your natural breast crease (or IMF).
